Care Manager LCSW - Case Management

Providence Holy Cross Medical Center in Mission Hills, CA is seeking a Care Manager LCSW for their Case Management department. This is a part-time position working 8-hour Day shifts.

About the Role

The Inpatient LCSW Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Providence Holy Cross is a four-time Magnet®-designated hospital — the highest recognition for nursing excellence.

Responsibilities

• Assess patients for social, financial, cognitive/behavioral, and legal barriers to care
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Coordinate discharge planning and connect patients with community resources
• Provide crisis intervention and brief supportive counseling
• Ensure continuity of care for vulnerable patients
• Document all interventions and ensure regulatory compliance
• Advocate for patients and families throughout their care journey

Required Qualifications

• Active California LCSW license
• MSW from an accredited program
• 1+ years of social work experience in a healthcare or acute care setting
• Knowledge of discharge planning, community resources, and case management
• Strong communication and organizational skills

Preferred Qualifications

• Experience in an acute inpatient setting
• Case Management Certification (CCM or ACM)
• Experience with Epic or similar EMR
