Care Manager RN - Case Management

Providence Holy Cross Medical Center in Mission Hills, CA is seeking a Care Manager RN for their Case Management department. This is a full-time position working 8-hour Variable shifts.

About the Role

The Inpatient RN Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Providence Holy Cross is a four-time Magnet®-designated hospital — the highest recognition for nursing excellence.

Responsibilities

• Perform comprehensive assessment and planning for high-risk patients
• Coordinate care across interdisciplinary teams throughout the patient stay
• Manage resource utilization and conduct level-of-care reviews
• Develop and coordinate discharge planning, connecting patients to post-acute resources
• Document all interventions and ensure regulatory compliance
• Advocate for patients and families throughout their care journey
• Collaborate with physicians, social workers, and community resources

Required Qualifications

• Active California RN license
• BSN preferred
• 2+ years of acute care clinical nursing experience
• Knowledge of care management, utilization review, and discharge planning
• Strong communication and organizational skills

Preferred Qualifications

• Case Management Certification (ACM, CCM)
• Experience with Epic or similar EMR
• Knowledge of Medicare/Medi-Cal criteria and InterQual/Milliman guidelines
