Social Worker, Full-Time Evening Shift

About the role

Adventist Health Glendale is one of the area's leading healthcare providers, comprised of a 515-bed hospital, two urgent cares, home care services, comprehensive cardiology care, and a vast scope of services in the San Fernando Valley.

Provides services to patients and their families to deal with needs incidental to their hospitalization, including counseling, support, and assistance in making transition plans.

What you'll do

• Collaborate and facilitate patient discharge plans for those requiring post-Hospice or post-inpatient care
• Assist patients/families/caregivers in determining the appropriate level of Hospice care; refer to outpatient resources as appropriate
• Initiate and reassess a plan of care based on the patient's physical and emotional needs
• Develop a Plan of Care based on patient assessment, including psychosocial factors, stages of human development, impact of terminal illness, mental status, and family dynamics

Requirements

• Master's Degree in Social Work required in CA
• One to two years' experience in an acute or long-term medical setting preferred
