About the Role

Provides excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and the patient's healthcare benefactor to optimize outcomes. Coordinates a continuum of care for a defined population of patients from pre-admission through post discharge.

What You'll Do

• Lead coordination of patient care with other disciplines on the care team, monitoring appropriateness and timeliness of care
• Ensure the interdisciplinary care plan matches the patient's clinical course, continuing care needs, and covered services
• Discuss resource utilization, consultations, treatment plan, length of stay, and discharge plan with physicians — with a focus on complex patients, frequent ED utilizers, chronic pain, substance abuse, and homeless patients
• Coordinate transfers to tertiary centers, including transfer of patient information for continuity of care
• Provide oversight and collect data required for regulatory and accreditation compliance

Requirements

• Bachelor's Degree in Nursing (BSN) preferred; experience in a care management role preferred
• Registered Nurse (RN) licensure in the state of practice (required)
• Case management certification preferred; BLS certification required
