Medical Social Worker II - Panorama City (Per Diem)

About the Role

The Medical Social Worker II provides assessment, coordination, and implementation/evaluation of professional social services to patients and families coping with social, emotional, and economic difficulties related to medical problems — handling more complex cases such as crisis intervention and behavior modification.

What You'll Do

• Perform bio-psychosocial assessment interviews with patients, families, and significant others
• Provide crisis intervention/treatment to adults, children, and families in emergent situations
• Interpret patient condition and recommend appropriate psychosocial intervention and treatment plans
• Provide patient/family advocacy and education on psychosocial adjustment to medical illness
• Utilize knowledge of KP and community resources consistent with treatment goals
