Care Manager MSW – Case Management

About the Role

Providence Holy Cross Medical Center in Mission Hills is seeking a full-time Care Manager MSW for Case Management (8-hour day shifts). Holy Cross is a four-time Magnet® designated hospital—one of just four in California. The Inpatient MSW Care Manager provides patient-centered care management for at-risk patients, including assessment, coordination of care, discharge planning, and patient advocacy.

What You'll Do

• Assess patients and plan comprehensive care management
• Coordinate care and discharge planning for vulnerable patients
• Identify resources to address social, financial, cognitive/behavioral, or legal barriers to care access
• Document interventions and ensure regulatory compliance
• Advocate for patients throughout their hospital stay

Requirements

• Master's Degree in Social Work from an accredited CSWE program
• 1 year of social work experience in any healthcare setting
• 1 year of experience in an acute care setting (or completion of TIPs/Case Management Orientation Program)
• California Fire and Life Safety Card within 30 days of hire
