Care Manager LCSW – Case Management

About the Role

Providence Holy Cross Medical Center in Mission Hills is seeking a part-time Care Manager LCSW for Case Management (8-hour day shifts). The Inpatient Licensed Clinical Social Worker Care Manager provides patient-centered care management services for at-risk patients in an acute care environment—covering assessment and planning, coordination of care, discharge planning, documentation, and patient advocacy.

What You'll Do

• Assess and plan care management for at-risk inpatients
• Coordinate care and discharge planning for vulnerable patients
• Identify and address social, financial, cognitive/behavioral, or legal barriers to care access
• Document interventions and ensure regulatory compliance
• Advocate for patients throughout their care journey

Requirements

• Master's Degree in Social Work from an accredited CSWE program
• California Clinical Social Worker License upon hire
• 2 years of social work experience in any healthcare setting
• 1 year of experience in an acute care setting (or completion of TIPs/Case Management Orientation Program)
• California Fire and Life Safety Card within 30 days of hire
