About the Role
Provides excellent patient care through collaboration, development, implementation, and reporting on the case management program. Acts as liaison between patients, families, nurses, physicians, and multidisciplinary teams to optimize outcomes. Coordinates care from pre-admission through post-discharge.
What You'll Do
- Lead coordination of patient care with other disciplines, monitoring appropriateness and timeliness
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course and covered services
- Discuss with physicians: resource utilization, consultations, treatment plan, estimated length of stay, and discharge plan
- Focus on complex patients, frequent ED utilizers, chronic pain patients, substance abuse patients, and homeless patients
- Coordinate transfer of patients to tertiary centers
- Manage frequent ED visitors via focus study reviews
Requirements
- RN licensure in California: Required
- BLS certification: Required
- Case management certification: Preferred
- BSN: Preferred
- Care management experience: Preferred
