Patient Care Coordinator (Outpatient)

Adventist Health Glendale in Glendale, CA is seeking a Patient Care Coordinator for outpatient services. This is a part-time position. Adventist Health Glendale is a 515-bed hospital with Magnet® designation from the American Nurses Credentialing Center.

Responsibilities

• Coordinate patient care activities across outpatient clinics and services
• Schedule appointments, procedures, and follow-up visits for patients
• Serve as the primary point of contact for patients navigating the healthcare system
• Communicate with clinical teams regarding patient needs and care coordination
• Assist patients with referrals, authorizations, and insurance-related matters
• Document care coordination activities in the EHR
• Support care transitions and discharge planning from acute to outpatient settings
• Educate patients and families about their care plans and available services

Required Qualifications

• High school diploma or GED; Associate's degree preferred
• 1+ years experience in a healthcare coordination, scheduling, or patient services role
• Strong communication and organizational skills
• Proficiency in EHR systems and Microsoft Office

Preferred Qualifications

• Medical Assistant or CNA certification
• Experience in outpatient clinic coordination
• Bilingual (English/Spanish) preferred
