FSP Case Manager

About the Role

FSP (Full Service Partnership) Case Manager at Child & Family Guidance Center, a nonprofit mental health organization in Northridge providing services to children, youth, and families in the San Fernando Valley.

What You'll Do

• Provide intensive, community-based case management to FSP clients
• Coordinate a full range of mental health and support services
• Conduct outreach and engagement with clients and their support systems
• Collaborate with healthcare, educational, and social service partners
• Maintain documentation in compliance with DMH and agency standards

Requirements

• Bachelor's degree in social work, psychology, or related field
• Experience in mental health case management or community services preferred
• Knowledge of LA County DMH Full Service Partnership programs a plus
• Bilingual English/Spanish preferred
