About the Role

Oversees Enhanced Care Management (ECM) program supervision (45%), documentation/compliance/reporting (30%), and program operations/collaboration (25%), including RNs allocated to Head Start contracts.

What You'll Do

• Provide program oversight and supervision for the ECM team
• Review health and dental records for children in the Early Care and Learning Program for CCL/OHS compliance
• Provide telephonic and virtual nursing consultation to families on child health concerns
• Train Early Care and Learning staff on child health conditions and related procedures
• Serve on the Division Leadership Team to ensure compliance with agency policy

Requirements

• BSN required; active CA RN license
• 3+ years of care coordination/case management experience with SMI, SUD, or child-welfare-involved populations
• Public Health Nurse certification (or obtain within 6 months)
• Bilingual English/Spanish required
• Up to 25% travel within LA and San Bernardino Counties

Schedule

Full-time, typically M–F with some early morning/evening/weekend flexibility. Field-based.
