- 2 years of Case Management experience (Care Coordination or Utilization Management), or completion of the Transitions in Practice (TIP) program
Preferred
- Master's degree in a healthcare-related field
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification
