Patient Navigator I - ECMP

About the Role

Full-time Patient Navigator I at Northeast Valley Health Corporation, supporting the Empanelment and Care Management Program (ECMP). Help patients navigate care, manage chronic conditions, and connect with community resources.

What You'll Do

• Guide patients through the healthcare system and help them access needed services
• Coordinate follow-up care and assist with care plan adherence
• Connect patients to community resources, social services, and specialty care
• Maintain accurate care management documentation

Requirements

• Experience in patient navigation, community health, or healthcare coordination
• Bilingual (Spanish/English) strongly preferred
• CHW certification or willingness to obtain
