About the Role
Telephonic Nurse Case Manager II at Elevance Health, based out of the Woodland Hills, CA office. Remote-eligible with required in-person training. Sign-on bonus: $2,000. Hours Monday–Friday 9:00am–5:30pm, with 1–2 late evenings depending on time zone. Multi-state licensure required (services members across states).
What You'll Do
- Assess, develop, implement, and monitor care plans for members with complex/chronic care needs, performed telephonically
- Facilitate authorizations/referrals within benefits structure
- Coordinate internal and external resources to meet member needs
- Interface with Medical Directors and Physician Advisors on treatment plans
- Negotiate reimbursement rates as applicable
Requirements
- BA/BS in health-related field; 5+ years clinical experience
- Current, unrestricted RN license; multi-state licensure may be required
- Case Management certification and managed care experience preferred
Pay & Schedule
$41.84–$62.76/hr. Full-time, remote (Woodland Hills, CA based).
