About the Role

What You'll Do
- Provide quality case management to health plan beneficiaries, assessing and supporting member health outcomes
- Assist members with medical needs, resources, social determinants of health, and safe care transitions
- Coordinate with internal and external partners on member care
- Conduct clinical assessments to develop and implement care plans
- Monitor and evaluate effectiveness of care management plans
Requirements
- HS diploma or equivalent; 3+ years acute care/condition-specific clinical experience; 1+ year as Nurse Care Mgr I or equivalent
- Current, unrestricted RN license (multi-state licensure may be required)
- Bilingual Spanish preferred; Case Manager certification preferred
Pay & Schedule
$41.84–$52.30/hr. Full-time, remote (Woodland Hills, CA based).
