About the Role

Per Diem Care Manager RN at Providence Cedars-Sinai Tarzana Medical Center in Tarzana, CA, working 8-hour day shifts. Case Management is a collaborative practice with patients, caregivers, nurses, social workers, physicians, and the community, facilitating communication and care coordination through transitional care and utilization management.

Requirements

• Bachelor's degree in nursing (or must obtain one within 3 years of hire, per policy)
• California Fire and Life Safety Card within 30 days of hire
• 2 years of experience in a healthcare-related field (acute, ambulatory, post-acute, etc.)
• 2 years of experience in Case Management (care coordination or utilization management), or completion of the Transitions in Practice (TIP) program
• Preferred: Master's degree in nursing or a healthcare-related field
