Care Manager RN - Case Management

About The Role

Providence is seeking a part-time Care Manager RN for Case Management during day shifts. Case Management is a collaborative practice involving patients, caregivers, nurses, social workers, physicians, payers, support staff, and the community.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate care transitions and discharge planning across the care continuum
• Collaborate with physicians, nurses, and social work to achieve patient goals
• Facilitate communication between patients, families, and the multidisciplinary team
• Ensure appropriate utilization of healthcare resources and services
• Document care coordination activities in accordance with regulatory requirements

Qualifications

• Current California RN license required
• Case management or discharge planning experience preferred
• Strong communication and critical thinking skills
• CCM certification preferred

Schedule

Part-time, day shifts.
