Care Manager MSW at Providence Holy Cross Medical Center in Mission Hills, CA. Part-time, 8-hour day shifts.

The Inpatient MSW Care Manager provides comprehensive patient-centric care management services for at-risk patients in an acute care environment — assessment and planning, coordination of care, discharge planning, documentation, regulatory compliance, and patient advocacy — ensuring continuity of care by addressing social, financial, cognitive/behavioral, or legal barriers.

Requirements

• Master's degree in social work from an accredited CSWE program
• California Fire and Life Safety Card within 30 days of hire
• 1 year of social work experience in any healthcare setting
• 1 year of acute care experience, or completion of TIPs/Case Management Orientation Program
• Preferred: acute medical-surgical terminology knowledge, prior case management experience
