The Inpatient LCSW Care Manager provides comprehensive patient-centric care management services for at-risk patients in an acute care environment — assessment and planning, coordination of care, discharge planning, documentation, regulatory compliance, and patient advocacy — ensuring continuity of care by addressing social, financial, cognitive/behavioral, or legal barriers.

Requirements

• Master's degree in social work from a CSWE-accredited program
• Upon hire: California Clinical Social Worker License
• California Fire and Life Safety Card within 30 days of hire
• 2 years of clinical social work experience in a healthcare setting
• 1 year of acute care experience, or completion of TIPs/Case Management Orientation Program
• Preferred: acute medical-surgical terminology knowledge, prior case management experience
