Social Worker Full-time, Evening Shift

About the Role

Adventist Health Glendale — a 515-bed, Magnet-designated hospital between Burbank and Pasadena — is hiring a Social Worker for the evening shift, full-time.

What You'll Do

• Provide services to patients and families to address needs related to hospitalization
• Provide counseling, support, and assistance with transition plans
• Assist with planning and post-discharge coordination with outside agencies, nursing staff, and relatives
• Collaborate on discharge plans for post-Hospice or post-inpatient care
• Develop and reassess plans of care based on psychosocial and clinical factors

Requirements

• Master's Degree in Social Work (required in CA)
• 1–2 years' experience in an acute or long-term medical setting preferred
