About the Role
The Outreach Care Specialist ensures that appropriate member treatment plans are followed on less complex cases and proactively identifies ways to improve members' health and meet quality goals. Virtual role (California-based), with required in-person training sessions; schedule 8am-5pm PST.
What You'll Do
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling members in programs
- Assess member compliance with medical treatment plans via telephone or on-site visits
- Identify barriers to plan compliance and coordinate resolutions
- Recommend treatment plan modifications in conjunction with case management and providers
- Coach members on ways to reduce health risks
Requirements
- High school diploma or equivalent and minimum 1 year of related experience
- Certified nurse assistant, certified medical assistant, or related BS/BA preferred
- Bilingual candidates preferred
