Responsible for comprehensive patient data collection as the foundation for care planning, overseeing accuracy and completion of the IRF-PAI assessment and coordinating the interdisciplinary plan of care, intake, and discharge planning.
Schedule
Full-time, 8-hour day shifts.
Requirements
- Degree in LVN, RN, PT, OT, or SLP, or a Bachelor's degree
- California Registered Nurse License (upon hire)
- 5 years in a hospital environment; 3 years of rehab-related experience
- Case management/PPS Coordinator experience and medical coding certification preferred
