About the Role

Full-time Care Manager RN. Variable 8-hour shifts. Case Management is a collaborative practice facilitating communication and care coordination through effective transitional care management and utilization management.

What You'll Do

• Facilitate communication and care coordination across the care continuum
• Conduct utilization management and transition of care planning
• Collaborate with patients, caregivers, physicians, social workers, and payers
• Document care management interventions and outcomes
• Advocate for appropriate, cost-effective care

Requirements

• RN licensure in California
• Case management or care coordination experience preferred
• Strong communication and interdisciplinary collaboration skills
