Care Manager RN Full-time Day

About the Role

Care Manager RN at Providence Saint Joseph Medical Center in Burbank, CA. Full-time, 8-hour Day shifts. Case Management is a collaborative practice coordinating care across patients, caregivers, nurses, social workers, physicians, and the community through effective transitional care management.

Requirements

• Bachelor's Degree in Nursing preferred (RNs without a BSN must obtain one within 3 years of hire, per policy/contract exceptions)
• California RN License upon hire
• 2 years of experience in a healthcare-related field
• 2 years of experience in Case Management or completion of the Transitions in Practice (TIP) program

Preferred

• Master's degree in a healthcare-related field
• Accredited Case Manager (ACM) or Certified Case Manager (CCM) upon hire

Schedule

• Full-time, 8-hour Day shifts
